Instructions for Requesting a Dog License
Electronically: Choose which license you would like to purchase then click PAYMENTS: after payment has been processed go to CALDWELL DOG LICENSE FORM “below”, complete form and submit.
Please provide a copy of rabies vaccination and alteration by fax 454-3640 or e-mail to petlicense@cityofcaldwell.org
By Mail: Complete and send printable form to:
