NOTE: Alcohol Beverage Catering Permit applications MUST BE SUBMITTED AT LEAST 5 BUSINESS DAYS before the event.

UPDATED APPLICATION FORMS:

1. Click on button on the left, then download the form to your computer or
mobile device. Download button is in the upper right corner when you
are viewing the form.
2. Open the file you downloaded -- it will now be fillable on your device.
3. Fill out the form, then click the 'submit' button near the bottom of the form.
This will open an email window. Attach any required items, then select 'send'.

1. Click on the button on the left; you will then be able to see the form.

2. Use the print icon in the upper left portion of the window to print the form.

3. Fill the form out by hand, and scan/email it to cityclerk@cityofcaldwell.org, or
bring it to 205 S 6th Ave (City Hall), and turn it in to the City Clerk's Office.
